About 5% of the time, a Pap may collect insufficient cells – for those, we
will have you come in for a second Pap. Due to contractual obligations
with your insurance company, we will require a copay at that visit.
Medical Records Release
In order to release your medical records, we must have your request in writing. You may come to either office and complete a medical records release form, or you may send us a letter by mail or send a request by fax to our Lebanon office at 717.273.0728.
In your request, please include the reason as well as where you would like the records sent, your date of birth and a daytime phone number where we can reach you if we have any questions. A signature is required for release of medical records. If they must be sent and received by a certain date, please let us know this as well.
Once the release is received, it can take up to two weeks before the records are ready. There may be a fee for the release of your records. We follow state guidelines for charges associated with the release of medical records. If there will be a charge, a staff member from our office will contact you directly.
Once copies have been made, the records must either be mailed to you or your new physician or they must be picked up in our office. This is for your own protection due to the confidential nature of your medical information.
