


Medical Records Release Instruction
In order to release your medical records, we must have your request in writing. You may come to either office and complete a medical records release form, or you may send us a letter by mail or send a request by fax to our Lebanon office at (717) 273-0728.
In your request, please include the reason as well as where you would like the records sent, your date of birth and a daytime phone number where we can reach you if we have any questions. A signature is required for release of medical records. If they must be sent and received by a certain date, please let us know this as well.
Once the release is received, it can take up to two weeks before the records are ready. There may be a fee for the release of your records. We follow state guidelines for charges associated with the release of medical records. If there will be a charge, a staff member from our office will contact you directly.
Once copies have been made, the records must either be mailed to you or your new physician or they must be picked up in our office. This is for your own protection due to the confidential nature of your medical information.

Yes. All of our providers see patients in both office locations. All of our charts are stored in our Lebanon Office and are transported to Palmyra daily for scheduled appointments. Routine appointments may be requested at either location. If you are having a problem, however, and request a specific provider, the first available appointment may not be available at your location of choice.
Yes. All of our providers are accepting new patients at both locations. Dr. Paterniti, Dr. Clark, Dr. Christman are all GYN-only and are accepting only GYN patients.
Pennsylvania Act 68, which went into effect January 1, 1999, now allows women direct access to obstetricians and gynecologists. Some restrictions do exist for self funded plans. Therefore, in most cases, you do not need a referral for either an annual examination or a problem visit, and you may schedule your visit with us directly. Please contact your insurance company directly for your plan's requirements.
Blood can be drawn in the Hyman Caplan Pavilion in a separate office or at a blood draw location of your choice.
